| 11 – 50 employees | $ 200.00 plus HST |
| 51 – 100 employees | $ 350.00 plus HST |
| 100 + employees | $ 450.00 plus HST |
| Retired | $ 55.00 plus HST |
| Student | $ 25.00 plus HST |
